Offices of the Department of Community Health
  Time frame for Processing:
One hour, then application is referred to the Governor or the judge that has jurisdiction over closure

Authority:
Public Health Law, and Trades and Industries Law

Filing Procedures: 

Filing of application at the department offices
Signature of Director then referral to responsible division
Field inspection is conducted and the inspection report is signed by Director
Message is conveyed to responsible authority to take appropriate action
